It's a day deep into autumn, the last day of October and Halloween. Haruhi Fujioka is greeted by Kimiko Sakurazuka, a regular guest of the Ouran Host Club. Haruhi greets her and continues walking until she sees a bright, shiny light

reflecting on her. Looking up, she sees a mysterious girl with long hair. Haruhi is later seen in Class 1A where she remembers a class event is to be held on Halloween, so she asks Kimiko and Momoka Kurakano if there are regular classes that day. Kimiko and Momoka explain that the last day of October is when the school’s mock exams are graded.

Momoka informs Haruhi that the school is allowing students to hold a Costume Ball and they leave in order to change into their costumes. Haruhi remembers the girl with long hair but before she can dwell on it, Hikaru Hitachiin and Kaoru Hitachiin proclaim, "Trick or Treat! We’re the Angel Brothers!”

Haruhi takes two candies from her pocket and gives them to the twins who eat their candy quickly and say “Trick or Treat!” again. Much to the twins’ dismay, Haruhi gives them more. Tamaki Suoh, dressed as a vampire, interrupts them to reveal that Haruhi’s blazer is filled with candies because he doesn't want her to endure the suffering he endured from the twins the previous year. He then recounts how, when he didn’t have candy to give to the twins, they wouldn't stop tickling him and might do the same to an unsuspecting Haruhi.

The girls flutter around Tamaki and he flirts with them. Meanwhile, the twins are disappointed. Hikaru notices Haruhi putting things into her backpack and asks what she's doing. Tamaki leaves with several girls in order to tour campus. Haruhi says she's going to the library if there's no class, but Hikaru scolds her about her attitude, saying that he's prepared a witch's costume for her. Haruhi remembers the girl with the long hair as being a witch or a student dressed as one.

Through a nearby window, she spies the girl with the long hair still standing on the top floor of the Central Hall, now grinning at her. When she announces this, Kimiko and Momoka whisper together. Hikaru says he can’t see anything, then asks Class President Kazukiyo Soga if he sees anything. Haruhi doesn't understand, but then the window cracks and Kimiko and Momoka scream that Haruhi is cursed.

Kazukiyo explains that the “Witch in the Central Hall Curse” is an Ouran Academy legend about a girl who dressed up as a witch, but fell from the Central Hall top floor years ago and that anyone who sees her is cursed

with everyone around that person to be sacrificed. Hikaru says that the story is stupid and impossible. Kaoru enters and explains that he tripped while exiting the bathroom. The belltower clock chimes that it's 11:40 a.m. with a weird sound. Momoka says it's due to the curse on Haruhi. Kazukiyo is scared. Nekozawa overhears and asks about the curse. Haruhi, still confused and somewhat disbelieving, tries to calm things down. Meanwhile, Tamaki has toured the ladies peacefully around the South Hall of campus.

She tries to leave, but the girls ask where she's headed. Nekozawa stops her to explain about curses, saying Haruhi is lucky. A wind then blows Nekozawa’s dark wig off, revealing his blond hair which he says is also a curse, then "returns" the curse to Haruhi. The twins find this funny and Haruhi is disappointed.  Renge Houshakuji appears, running over to see if Haruhi is okay. Haruhi observes that Renge’s costume appears to be a school uniform from pop culture. Renge goes into "otaku mode" to explain that her costume is the uniform for the female characters in “Thumping Heart Memorial 2” and that she paid money to research and locate it. Haruhi is unimpressed.

Renge then asks her to join her and the other girls to make lanterns and banners. Haruhi refuses since she's "under a curse” and is, instead, going to the library. Kimiko, Momoka and Renge fangirl about Haruhi worrying about them.

Hikaru pinches Haruhi's cheeks.

They explain that they just want cheer her up, so Haruhi agrees to help them. They go to a Cooking Room where Haruhi attempts to teach them how to bake cookies. While searching for salad oil, she sees the witch with the long hair in a reflection. Frightened, she turns and bumps into Hikaru. Haruhi tries to tell him about the reflection, but Hikaru pretends not to understand before pinching Haruhi's cheeks and calling her a "chicken" with a smirk. When he lets go, Haruhi asks about Kaoru and is told he's in the infirmary. Hikaru talks about the day's events, the Host Club and the Costume Ball. Haruhi is shocked to think about everything happening on the same day.

Hikaru starts to take one of Haruhi's cookies, but she removes his hand and says that he's a pest and a punk. Hikaru retorts that he's trying to help her and to include Kaoru in her descriptions. Haruhi says they are different from one another in that when Kaoru is alone, he's calm and reasonable while Hikaru is always loud and unreasonable and Hikaru has a potty mouth. Hikaru is shocked that Haruhi notes their differences and he smiles. Haruhi corrects his thinking that her words are a compliment to which Hikaru requests that Haruhi praise him, too.

Kaoru enters the Cooking Room and talks about the scar he got when he tripped in the bathroom. The girls give Haruhi the last pumpkin to turn into a lantern, but Haruhi doesn't know how. With Kaoru's help, she finishes her lantern,

but her drawing isn't very good. Kaoru laughs while calling Haruhi’s drawing a masterpiece. Haruhi tells him to shut up. Kaoru says he'll carve Haruhi’s pumpkin, but still calls her amazing. The oven beeps and Haruhi takes out the cookies, which turn out well. Hikaru helps Kaoru carve Haruhi’s pumpkin and Momoka says that the twins look like they're enjoying themselves. Haruhi says that it's because it's been a day full of naughty twin tricks. Kimiko agrees but Momoka adds that despite their tricks, they're friendlier with everyone now. Renge praises Haruhi's good influence on the twins, as well as her excellent cooking skills as she cites the catchphrase, “I can eat three bowls of rice!”

Kyoya Ootori dresses up as a priest,

Kyoya, Mori and Honey

Mitsukuni "Honey" Haninozuka as a cat and Takashi "Mori" Morinozuka as a dog. They all come to the Cooking Room where Kyoya informs Haruhi that she’s needed for the Costume Ball as a Host Club member. Renge flutters over Kyoya and Haruhi while the other girls flutter over Honey and Mori. Honey approaches Haruhi and says, "Trick or Treat!” so she gives him some freshly baked cookies. Meanwhile, Hikaru says he's cut himself with a knife while carving the pumpkin, causing a commotion.

Haruhi recalls past events and notices that the twins are always involved, somehow.

The twins got found out

Haruhi is angry.

She also realizes that Halloween is unfolding just as the twins had planned, much like their fake fight. She goes with the twins to the infirmary and, taking Hikaru’s hand, washes it under the faucet but sees no wound. Hikaru smirks, so Haruhi asks about the person with them who is wearing a pumpkin head. It's Kazukiyo Soga, who blames the twins. Haruhi dashes to the Central Hall and finds Kaoru dressed up as the witch. Kaoru initially thinks it's Hikaru behind him, but finds an irate Haruhi when he turns around.

After a while, the Host Club gathers in Music Room 3 and Tamaki scolds the twins who cower on the ground.

The twins, however, actually look bored and an infuriated Tamaki demands that they apologize to Haruhi. They finally do so, embarrassed. Haruhi also recognizes her own attitude and invites them to go to the Costume Ball with her. And with that, they leave Music Room 3 and attend the event.

Trivia

Renge's catchphrase is from the anime Green Green.
